The Hospice Help Network is a place for individuals and families to come to get emotional support, education, and community resources as they consider or travel the path of hospice services. Hospice Help Network is not a replacement for the services provided by the amazing interdisciplinary hospice teams, but we hope to be a support and partner to the team, patients, families, and communities they serve.
We provide the following supports to individuals and families:
Education on hospice guidelines and general qualification criteria, services commonly offered by hospice providers, guidelines for Medicaid application procedures, the difference between levels of institutional care, different types of advance directives, and supportive services available in the community.
Emotional Support for issues related to anticipatory grief, the dying process, legacy projects, family dynamics, caregiver stress, denial, loss of independence and purpose, adjustment to having a terminal condition, coping with a terminal condition, management of disease progress, long-term care planning, and deciding whether or not to pursue hospice or palliative care.
Referrals can be made to local hospice and palliative care programs, death doulas, elder law offices, funeral homes, in-home caregiving agencies, financial advisors, and local supportive community services that assist with elder care, disability, financial assistance, or any other support relevant to client needs.
We provide the following supports to communities:
Online presence to raise awareness and provide education on topics regarding end of life, hospice, long-term care planning, and available community resources.
Public educational seminars, lectures, and webinars on the same topics.
